what arrest & DUI conviction details to add in DS160 form

Hi All,

I was arrested for DUI in Aug 2017( BAC .14) and Plead Guilty to charges so was convicted for DUI in May 2018.

Around May 2018, i left my existing Employer as I found new employer and they agreed to Sponsor me for H1B, so H1B transfer was initiated. I received the approval for working with the new employer, before my conviction date in May 2018 end. 

I traveled to India a week after my conviction in June 2018, now I plan to go for stamping, what details should I mention in the DS160 form and what court documents I need to take during visa interview.

Also, since my employer created 3 months SOW, i received 3 months approval and considering 50 days wait time in Indian embassy and 3-4 weeks time for physical examination, is it advisable to go for visa interview?
