Original or renewed passport details for NORI affidavit/bio data (India)

Hi everyone!

I'm about to get my documents attested to start the NORI application process. My passport that I came to the U.S. with has expired, and I got my passport renewed (in the US). For the application form, bio data and affidavit should I put details of both passports or should I just put one? If I just put the new one I'm concerned the Regional Passport Office in India might be confused when I ask them for the clearance. 

Also, I know that I can't change/extend my program/DS-2019 once I apply for the waiver with the Department of State. Can I still change/extend it while applying for the Indian part of the waiver?

Any help would be greatly appreciated! Thanks!

From other forums after rfe usually it takes 6 to 8 weeks to hear back after they receive the requested documents.

I am in the same boat applied early november still nothing yet.

In case this is helpful to anyone, I checked with the embassy and they wanted the current passport details on the affidavit and bio-data.
