DS160 - H4 with both SSN & ITIN

Filling out DS160 for H4 who has both SSN & ITIN now. ITIN will be expiring soon. Do we need to mention both SSN & ITIN in the DS160?

You cannot have both at the same time. After getting SSN you've to get the ITIN cancelled by mailing IRS.
