H1B transfer and travel post approval

Can one travel to home country post approval from new employer , say -

H1b visa approved and stamped -  Company A (current company)

H1b visa approved - Company B (new company)

Now, going on vacation from Company A , returning back to Company A -> working for couple of months --> is joining company B is possible i.e. will petition approval for company B still be valid ? 

second question - company A initiated h4 visa, say h4 visa will be stamped during this vacation --> spouse travel to US during this vacation with company A question is -- does company B needs to initiate H4 visa transfer for spouse ? does this process needs to be completed before joining company B?

Please remember visa and petition are different entities. The H4 visa is based on your valid H1 status with the current employer. If you change to B then file her H4 extension based on the petition approval with B after she comes over.

Regarding first question, my concern is whether we can join company B after vacation (home country)  and working with company A for sometime? Please note approved petition on company B is obtained before vacation. Does i-94 on this B company petition will not be affected after rentry to US?
