H4 EAD

Hi All

My spouse was approved for an H4 EAD during March,2017 ( Got approval till Oct-2017) , but USPS misplaced the EAD card and notified the same to us in May 2017. Once notified we immediately reapplied.

My VISA status was valid October 2017. The second time around the process took a little longer and as my VISA was done by October 2017, the H4 EAD got denied as well.So we don't have the EAD Card.

I am again having a valid VISA until March,2019. I want to apply for the H4 EAD again.

My Questions are

a) If i need to apply myself which option do i need to select in I 765 Form

            i) Permission to accept employement

          ii) Replacement 

           iii) Renewal of my Permission to accept employment.

b)  In I765 Form, for question 15 , "Have you ever before applied for employement authorization from USCIS? " for this question , what do we need to answer?

Please advise or provide your inputs.
