H4 EAD clarification

Greetings,

I’m filing H4-EAD based on my husband’s approved I-140. I need your help on below queries:

• I entered in US on L2 visa however later on my husband’s status changed to H1. Therefore currently I’m on H4 visa. In this case I’ll have to write “Status at Last Entry” as L2 and current status as H4 in I765 form. Is that correct?

• Do I need to provide any additional documents for L2 visa status?
