Past I140 Satus Check

Hi, 

My past employer 'A' applied for I-140 in Sep 2013 and got a receipt notice (I 797C, Notice of Action) with a notice date of Sep 2014. 

I left that employer 'A' in Jan 2015 and when I checked the case status recently it says " in July 2015, USCIS sent the case, Receipt Number XXX , to the Department of State for visa processing. You can check the status of your case at the National Visa Center after six weeks from the above date by visiting www.nvc.state.gov". 

Now my employer 'B' is filing for I-140 and there are questions on past info like - "Have you ever applied for an immigrant visa to obtain permanent resident status at a U.S. Embassy or U.S. Consulate abroad?" 

I am confused on what to answer for it. As such I know the answer is 'Yes' but am not sure what is the decision status - I don't have a clue if it was withdrawn or denied etc. 

Is there a way to check about that ? How to check the status at NVC.state.gov ?
