L2 EAD expired and lost, want to apply H4 EAD

Hello, I need your help with the situation I am in currently. My wife was on L2 and we applied for her L2 EAD in 2009, which was approved. She never used that EAD as she never started working. She changed the status from L2 to H4 in 2012 as I changed from L1 to H1B. She has been to India and has valid H4 stamping as of today. 
We are planning to apply her H4 EAD as she now wants to start some work. Our problem is, we are not able to find L2 EAD card or any other documentation related. I only have petition number (WAC...).
Now while filling the H4 EAD form, should we say YES to the question 11 asking Have you ever applied for ead before or say NO. If saying YES, how would I furnish the documentation?
