H1B Extn : Roles and responsibilities changed from original filing

Here is the context :

My stamped H1B visa has expired (11/2016) and I have an approval (I-797) till 11/2017. In the meanwhile, I have progressed in my career from an IT consultant to an IT trainer. As I am a trainer now, I teach others to do the job that I did until few months back. So, my role has changed significantly.

My Questions :

1. Can H1B extn be filed with an amendment to reflect the new role change and increase in pay? Are there any implications?

2. As an IT Trainer, the training is all over U.S. How does that impact the LCA? How can I file it differently?
