Transfer to H1B from H4

Did residency in Hospital ‘A’ for 3 years on H1B-visa. After which switched to H4-EAD in 2016 and working now as a doctor in Hospital ‘B’.

If I were to get an H1B via Hospital ‘B’, would I fall under the lottery/cap? i.e would it be considered like a new H1B where I would have to apply in April and be subject to the lottery

 

OR would it be considered like a renewal where I could apply for H1B at any point in the year?

You can apply H-1 at any point.
