Fellowship with EB2-NIW

I have an approved physician NIW with PD of May 2010(India) and I currently work as hospitalist. I have matched to a fellowship program and intend to start next year in July 2016 on H1b visa. I have some questions/concerns based on the visa bulletin trends.
1) Will I be able to do AOS(based on approved NIW) during fellowship( if PD becomes current after July 2016) without requiring any paperwork from my fellowship employer?

2) If my PD becomes current before July 2016 or if I become eligible to file for AOS am I subject to AC21 rule to wait for 180 days after EAD application to do any change of employment. 

3) I had applied for NIW based on work in HPSA from August 2010 until now.I have not gotten any communication from USCIS regarding requiring anymore work in HPSA therefore I presume I got retroactive credit for all 5 years that I requested.Am I correct in assuming so.If not pl advice my next steps.
