EB1-A employer sponsored - job change?

Hello,

 

EB1A (ext. ability) approved, but was sponsored by the current employer (with physician listed as a beneficiary). Can I change jobs (same field) before, or at the time of, filing I-485, or is the fact that the employer sponsored I-140 (even though it's not required, and no PERM was needed), prevents me from doing so?

If it indeed is EB1A then it did not need a job offer. You should firm up your options by a consult with say the firm of Murthy or the original firm which filed your paperwork.
