URGENT : DS160 question on Have you ever been unlawfully present, overstayed the amount of time granted by an immigration official

While filling DS160, I came across the following question for which I am not sure on what to answer.  Please find my complete case details and advise me what to put on DS160 application for the following question.

 

Have you ever been unlawfully present, overstayed the amount of time granted by an immigration official or otherwise violated the terms of a U.S. visa”

 

My Case details: H1 extension filed somewhere around march , I94 is valid until May 1 2014

                                  H1 (ext) Denied on Jun 15 2014

                                Employer got denial letter on Jun 20

Employer told H1 denied due to some missing documents and he refilled H1 on Jul5th (At this time my I94 expired and my employer told that  there is one clause where in you can apply for new H1 requesting USCIS to issue I94 starting from previous I94 expiration rate)

                                Got RFE on Jul 14

                                RFE answered

                                H1 got denied again on Aug 13

                                Left US on Aug 24 2014

 

If the Answer is Yes, Please advise me on what to write in details section? 

 

Will I get any issues in my Visa Stamping? Please help..

 

Generally what questions they ask at consulate for my case..

 

Please advice..
