Is new H1B Visa stamp required If transfer has been applied with out entering usa.

I have been working with Company A.

I have my H1B petition approved from Company B. Got visa stamp in September 2014. Company A want to retain me and hence applied for H1B transfer even with out travelling to USA on Company B stamp. 

Company A H1b petition is also approved in december 2014. 

 

Questions

1. Do i need to go to visa stamping again for Company A as I have never entered USA or worked for Company B. 

2. In case if I go for the stamping are there any chances that the consulate officer will reject both the visas. 

3. Will this run into 221's in case If I need to go for stamping.

4. Will there be any issues if I got with Company B instead of Company A as I dont want to go for stamping again?
