2nd h1 stamping and ds160 filling help

Dear Forum members, 

 

  I got my first stamping in Vancouver and now have booked an appointment in Jamaica for the 2nd stamping. While filling the DS 160, I came across the following questions which my other friends were not so sure about as well.. please advise on the following:

 

1. Since my first H1 stamping was done in Sep 2012 in Vancouver, now while filling the DS 160, should I say "Yes" for ten-printing of fingerprints question?

 

2. My employer has filed for i-140 and received receipt but not approved yet. The question regarding has anyone filed immigration petition with USCIS - should this be yes? It also gives an explain section: what should be written here?

 

3. Should spouse be mentioned on the immediate relatives page of DS 160 as there is a separate page asking for Spouse details later?

 

4. Should spouse address in the drop down be home address as she stays with me in US or address on her passport( which can be filled using "Other" from drop down)

 

Murthy forum has been immense help during my past stamping experience.. thank you in advance for taking time to answer my queries.

Thank You for the response. I did my first H1 stamping in Vancouver in Sept 2012.. I do not remember whether it was regular finger prints or ten-printed.. Any clue if in 2012 for H1 stamping they did ten printing?

It was all 10 finger prints.
