OPT working with multiple employers but only reported 1 employment

I am on OPT. My OPT started on Feb 7th of 2017.  I reported my 1st employment to my DSO on March 31, 2017. 

As I am employed by this employer. I got another job on April 18, 2017. But did not report this to DSO. From then I was working with both the employers. Recently I lost the job which I did not report. But I am continuing to work with the employer I reported to the DSO.

Is it necessary to report the start and end dates of the lost job at least now?

It is necessary to meet your DSO and tell him the facts and ask him for help as you were under a mistaken impression.
