H4 to F1 COS- documents required

Hi,

1. Can you please help what all documents i need to mail USCIS, when I am applying for H4 to F1 visa COS?

2. How many months bank statements are required to be shown? Is it sufficient to give a month's detailed bank statement, along with a bank letter stating how much funds we have. Or 3 months bank statements are required?

3. My spouse will be financing me. He will be signing the Affidavit of Support. I have a question, the Affidavit of support needs to be notarized? or is it okay if he just signs it?
