Applying for F1 OPT, Post COS from H4-EAD

My wife converted from her H4 to F1 to pursue her M.S. She had an H4-EAD, but was never employed. She's graduating this summer and applying for her OPT.  We find a question on the I-765 form:

"Have you ever before applied for employment authorization from USCIS?” 

 Are they talking about the H4-EAD card that she had earlier? should we answer “yes”? Do we have to provide any explanation/cover letter for the same? Would this be an issue in her getting an OPT?

Would appreciate your inputs on the same. Thanks a lot in advance.

Since you are aware she did apply for Employment Authorization then you should answer accordingly. No explanations necessary.
