I-140 Wrong Petitioner Name

I was trying to port my I-140 and found out that Petitioner's name on the form is wrong. My company's immigration officer's name, who signed on my application is mentioned as petitioner name instead of the company itself. 

 

1. Is it a common typo error ?

2. Can someone guide me about the procedure to get it fixed ?

3. Can my visa extension be impacted because of this error ?

Please note that I had filed for my wife's H-4 EAD which has already been approved.
