Addresses and Contacts in Reference letter

Hello Everyone,

 

I would really appreciate I have few question related to my Perm Application  which I have to provide information to my lawyer:

 

1)      I worked in 5 different companies. I don’t have my supervisors phone number for these places. Can I give companies board line in PERM application?

2)      I got letter experience letters from Chennai and Hyderabad the centralized HR for Accenture and CSC. However, my actual work place was Delhi and Noida. What shall I write in Job locations, it should be same as in experience letter?

3)      I got my masters from Hyderabad campus, However, the headquarter of my degree/university is moved to Dehradun, though other campuses are still in Hyderabad, what shall give for University address?
