COS F1 to H4 and H4 Extension

Hi Murthy Team,

My wife has approved H4 petition from 10/1/2015 to 02/01/2018.After this approval She went to India and came with F1-Stundent.

Now She completed her masters and got her OPT EAD approved from 10/3/2016 to 10/3/2017.Now she is working As fulltime employee.

She completed her masters in ACICS Accrediated college.ACICS lost Accrediation on Dec 12 2016.She cannot apply for OPT STEM extension.

Can you please clarify on below questions.

1.Can she apply STEM extension ?will it get approved?any other ways to get 24 month STEM extension.
2.Can she convert to H4 again.Can we use old H4 Approval notice for COS?Do weneed to send old H4 Approval notices also.
3.what is the right process,Can we convert F1 to H4 and Apply H4 EAD also Simultaniously.what is the right time to apply COS.Can we get both approvels at a time.
4.What is the best way to work with out affecting her present job.Any ways to do that.?
